While pain is common among seniors, it is not adequately treated or managed. In particular, pain in seniors with dementia is often undertreated and undermanaged. Although the undertreatment of pain among persons with cognitive impairments represents a serious ethical concern for pain clinicians, most writers in the area explain the undertreatment of pain by focusing on issues related to liability, fears of addiction to opioids, and erroneous beliefs that pain is a normal part of the ageing process. We argue that (...) the philosophical notion of personhood must also be given careful attention when considering the problem of pain undermanagement. In this paper, we consider the undertreatment of pain among seniors with dementia, while focusing on the ontology of relationships and on existential philosophy. Moreover, we outline a series of recommendations to help minimize the probability of pain undertreatment in this population and to encourage considerations relating to personhood when treating people with dementia. Heidegger’s two modes of thinking, calculative and meditative, were used as the thematic basis for this qualitative study of physicians from seven countries . Focus groups were conducted in each country with 69 physicians who cared for the elderly. Results suggest that physicians perceived ethical issues primarily through the lens of calculative thinking with emphasis on economic concerns. Meditative responses represented 24% of the statements and were mostly generated by Canadian physicians whose patients typically were not faced with economic barriers (...) to treatment due to Canada’s universal health care system. The purpose of this paper is to apply Aristotle''s theory of causation to the administrative realm in an attempt to provide the manager/student with a more complete basis for organizational analysis. The authors argue that the traditional approach to administrative case studies limits the manager''s/student''s perspective to the positivistic world view at the expense of a more encompassing perspective which can be achieved through the use of an Aristotelian approach. Aristotle''s four-part theory of causation is juxtaposed with contemporary views of (...) organizational ideology/philosophy, culture, climate and leadership, and staff or personnel. The Mazda automobile plant in Flat Rock, Michigan is provided as a sample case study to demonstrate the comprehensiveness of the Aristotelian method in organizational contexts. (shrink)

The ethical ideologies of psychologists and physicians were compared using the Ethics Position Questionnaire. The findings reveal that psychologists tend to be less relativistic than physicians. Further, we explored the degree to which physicians and psychologists report being influenced by a variety of factors in their ethical decision making. Psychologists were more influenced by their code of ethics and less influenced by family views, religious background, and peer attitudes than were physicians. He rightly suggests that “the story of childhood cannot be told in one-dimensional formulas of either innocence and vulnerability or unruliness and (...) undevelopment” (7), but instead require a new paradigm for ethics: “childism.” He questions three historical views of childhood: the “top-down,” the “bottom-up,” and the developmentalist. Wall splendidly moves beyond these views and demonstrates that considering children should instruct us in the limitations of ordinary approaches to ethics—not only for children’s sake but for adults too.How so? Wall’s constructive section considers ontology, teleology, and de-ontology. In them, relying heavily on twentieth-century continental philosophy, supplemented by some social-scientific literature and numerous anecdotal examples, he develops an overall approach to the moral life. First, he argues for an ontology of “restless and endless play” (36) reflective of the human capacity for creativity possessed by both children and adults. Secondly, the moral life has a teleology of “narrative expansion,” with a constant movement toward a “more richly diverse life story” (60). Finally, moral obligation involves a constant “decentering” of the self in which “the world I create for myself should be disrupted and stretched out as far as possible by genuine human difference” (88). Building on the insights of top-down and bottom-up traditions, and going beyond the “flatly unilinear assumptions” of developmental “stageism” (81) ethics involves a constant circularity, exemplified in the final three chapters on specific issues. For example, families who “create increasingly expansive family worlds” engage in top-down transmission of tradition, developmental formation of relationality, and bottom-up nurturing of otherness (149). These qualities apply not only to children but to parents as well, and thus satisfy Wall’s desire for an ethics that is fully “child-inclusive” and that does not privilege adult agents. He succeeds admirably at demonstrating that the real agency children actually possess and exercise, as well as their vulnerability and passivity, are not simply incomplete versions of what adults possess fully.I would raise two concerns. First, the book works so hard to avoid any kind of linearity that criteria enabling judgment seem lacking—for example, surely some [End Page 195] forms of “disruption” are not to be welcomed (especially by children). The telos of “narrative expansion” so strongly refuses to privilege any perspective that it threatens to empty out what one might gingerly call “adult commitments”—to stable love, production, civic life, and fine arts—all of which require virtue (absent here), and which seem necessary for children to flourish. Aspiring “to go on creating oneself endlessly” (58) can be a fruitful depiction of adult life, or it can be an immature escapism. Without a thicker telos, the distinction is difficult to make.Second, his category of “childism” (3) forces him to artificially avoid any telos of maturation. In parallel with other “isms” (e.g., feminism), he argues that children are “full human beings,” live in “fully meaningful moral worlds,” and are “full participants in... human moral experience” (29). “Full” appears throughout, but the inclusion parallel clashes with maturation in human life. Albert Borgmann described his work as “both inconspicuous and consequential”. As of this writing, he has no entries on The Internet Encyclopedia of Philosophy, The Stanford Encyclopedia of Philosophy, or even Wikipedia. Among those who know his work, most only know his book, The Inward Morning. And few of those who know the (...) book have seen fit to write about it. Ten years of educational reform have not brought dramatic improvements. In Ideology and Educational Reform, David Paris traces the underlying ideological problems that make genuine reform difficult. These include different and often conflicting beliefs concerning the proper role of public education as well as the public's natural ambivalence about schools as government agencies.Paris describes three major themes in public education—common school, human capital, and clientelism. There are several branches of ethics. Clinical ethics, the one closest to medical decisionmaking, can be seen as a branch of medicine itself. In this view, clinical ethics is a unitary hermeneutics. Its rule is a guideline for unifying other theories of ethics in conjunction with the clinical context. Put another way, clinical ethics interprets the clinical situation in light of a balance of other values that, while guiding the decisionmaking process, also contributes to the very weighting of those values. (...) The case itself originates ideas, not only about which value ought to predominate in its resolution, but also provides the origin of clinical rules that can be used in other cases. This overview of the "sister arts" of the nineteenth century by younger scholars in art history, literature, and American studies presents a startling array of perspectives on the fundamental role played by images in culture and society. Drawing on the latest thinking about vision and visuality as well as on recent developments in literary theory and cultural studies, the contributors situate paintings, sculpture, monument art, and literary images within a variety of cultural contexts. Field and Hineline have shown how pervasive and insidious is the tendency to make dispositional attributions, even among those who criticize the practice, and they identify a bias for models of contiguous causation as one reason for this tendency. They argue that order can be found at multiple scales of analysis and that in some cases a translation to a model of contiguous causation is impossible. I suggest that pragmatic considerations are sufficient to justify a particular scale of analysis and (...) observe that behavioral principles are fundamentally extended in time. However, I argue that accounting for variance is the goal of science; when events at one level are indeed mediated by those at another, more of the variance can be accounted for by considering both, and there is no principled reason for considering only one. Like most other sciences, behavior analysis adopts an assumption of uniformity, namely that principles discovered under controlled conditions apply outside the laboratory as well. Since the boundary between public and private depends on the vantage point of the observer, observability is not an inherent property of behavior. From this perspective, private events are assumed to enter into the same orderly relations as public behavior, and the distinction between public and private events is merely a practical one. Private events play no (...) role in the experimental analysis of behavior, but they permit us to make sense of many commonplace phenomena where controlled observation is impossible but unsystematic data are available. Moral capacity is an important feature of what it means to be human. In this volume, the contributors have taken on the daunting task of trying to distinguish between legal and moral capacity. This distinction is difficult at times for clinicians, philosophers and legal scholars alike. Part of the challenge of defining moral capacity lies in the difficulty of adequately categorizing it. For this reason, the editors have chosen to divide the book into three parts. As an introduction to this issue, I argue that the concept of autonomy is clearly important for many of the freedoms we enjoy. The problem in medicine with its use lies in interpreting the concept with respect to the impact of disease on persons, the models of medicine we employ, and the settings in which the problems arise. A short statement about the major points of the authors collected in this issue concludes the editorial.

Applying general and statistical knowledge to individuals is difficult either on epidemiological or epistemological grounds. This paper examines these difficulties from the perspective of computer registers of epidemiological data.

Relevance logic is ordinarily seen as a subsystem of classical logic under the translation that replaces arrows by horseshoes. If, however, we consider the arrow as an additional connective alongside the horseshoe, then another perspective emerges: the theses of relevance logic, specifically the system R, may also be seen as the output of a conservative extension of the relation of classical consequence. We describe two ways in which this may be done. One is by defining a suitable closure relation out (...) of the set of theses of relevance logic; the other is by adding to the usual natural deduction system for it further rules with ‘projective constraints’, whose application restricts the subsequent application of other rules. In this paper I address three problems posed by modern medical technology regarding comatose dying patients. The first is that physicians sometimes hide behind the tests for whole-brain death rather than make the necessary human decision. The second is that the tests themselves betray a metaphysical judgment about death that may be ontologically faulty. The third is that discretion used by physicians and patients and/or family in deciding to cease treatment when the whole-brain death criteria may not be met are (...) sometimes open to challenge. In each of these problems I find that the operative concept of death relates to life itself. This point is expanded by examining the uses of the word death in our language and culture. From these I formulate an initial ontology of death. In it, death is described through a relationship with life, rather than as an absence of life, of consciousness, awareness, or sensation. This ontology then leads to a proposal for an ethics of discretion about the discontinuation of treatment for comatose patients. (shrink)
